A fantastic opportunity (22.5 hpw) has arisen to join our Selby based community mental health service for older people.

The successful individual will need a Foundation Degree in Health and Social Care and Literacy and Numeracy Level 2 or equivalent to be recruited in to the position on Band 4.

The appointed person will be expected work with patients who experience mental ill health and often have a number of identified complex, and physical frailty needs.

The person will deliver patient care and carry out a number of clinical therapeutic interventions with limited or indirect supervision. The role of the candidate will be to work intensively with patients in their own home or care environment with the aim of improving mental wellbeing. The successful applicant should have an interest in physical healthcare in patients with mental health conditions, as the role will include physical healthcare monitoring.

Candidates will be enthusiastic individuals who have previous experience as a Band 2/3 or health care assistant; who can deliver recovery focused interventions and are keen to further develop their knowledge and skills. The successful candidate will be joining a motivated and supportive team, committed to delivering a high quality patient centred service and to working towards improving quality and effectiveness

Due to the nature of the role it is essential that applicants are able to travel independently across the geographical area covered by the team and have access to a suitable vehicle for business purposes.
