Due to an exciting expansion within our Portsmouth Rehab and Reablement Team (PRRT), we have a new opportunity for an Associate Practitioner to join our community urgent response service.
This role is for 25 hours per week.
PRRT are looking for dynamic and forward thinking health and care professionals to join our expanding integrated service. Your excellent clinical, interpersonal and team skills will contribute to the delivery of an integrated community care pathway to enable local people to improve, maintain or prevent a decline in health, supporting self-management of health problems and the maintenance of optimum quality of life and independence.
In return for your skills, we will offer regular supervision, internal and external learning and development opportunities.
The service operates from 7am – 10pm daily with all staff expected to participate in a rotating shift pattern. Applicants will be required to hold a driving licence and have access to appropriate transport.
