Children’s Community Nursing Team Leader – Band 7

An exciting opportunity has arisen within the Children’s Community Nursing Team within Birmingham Community Healthcare. The team service will works alongside the existing the Palliative Care team and the new Rapid Response Team. We provide clinical interventions and holistic assessments to children within their own homes 7 days per week and participate in an ‘on-call’ out of hours service. This post will combine operational team management, work in conjunction with the clinical co-ordinator, provide leadership and clinical practice The post holder will work closely with the clinical lead and service manager to ensure that a high quality service is delivered to children/young people and their parents/carers, and participate in service development and the development of clinical pathways across acute and primary care settings whilst maintaining a supportive team culture.

The emphasis of this service is to improve the quality of life for children and families with complex, life limiting and palliative healthcare needs, offering them greater choice around their chosen place of care and prevent hospital admissions and attendance

Qualifications:-

RSCN/ or RN Child Branch

Applicants must be a car driver.

Location: Lansdowne Health Centre

Grade: Band 7 x 1 fulltime substantive post.

Hours of work: 37.5 a week and participate in the on call rota.
