Bradford City and North Mental Health Team for Older People has a rare vacancy for a Care Coordinator. Based in the busy city of Bradford we cover the Bradford City and North areas of the city. We are a team which works both office based and in an agile manner which means after induction you will be able to start and end your working day at home or in the office.

We are looking to widen our team skills and experience, and would welcome applicants from allied health professionals with the relevant qualifications and experience, as well as registered mental health nurses.

We care for people over the age of 65 with any mental health problem and those of any age with dementia. We work closely with our inpatient wards to prevent admissions and across the Older People's pathway.

The team consists of, Nurses, Occupational Therapists, Physiotherapist, Psychologist and Assistant Psychologists, Registered Nursing Associate, Support worker, Therapy assistant, Community Engagement worker and Consultant Psychiatrists - Truly MDT

The team thrives on self and personal development, Supervision is key and you will be offered, clinical, caseload and personal 1-1 management supervisions to enable your best professional performance and practice.
