Job Description

We are looking for an enthusiastic registered Nurse to boost our existing Extended Primary Care Team as part of System Integrated Discharge Planning. Our services cover a wide geographical area including Fareham & Gosport, Havant and Waterlooville and Petersfield and Bordon.

These positions are designed to contribute to avoiding hospital admission, facilitating early discharge from hospital and maintaining patients in their own homes. This post offers the opportunity to work closely with all members of the MDT both in primary, secondary care, Adult Services, care agencies.

The roles are about coordinating care across organisations rather than referring on, following up with patients to see how they are getting on and lastly, it’s about bringing medical and non-medical professions together under one banner to provide proactive holistic care.

The successful candidates would complete a period of induction and will be required to assume responsibility for coordinating a plan of care, working within a scope of practice autonomously as a registered and non-registered staff member. The successful applicants will be expected to lead, under the supervision of senior team members, on care planning and identification of person centred goals. The applicant will work with the supervision and guidance of the band 6 and band 7 in the team. The applicant will need to possess excellent communication skills and be competent in linking with the larger multidisciplinary team. There will be the opportunity to develop service development skills, including supervision.

The successful applicant will be proficient, knowledgeable and competent. They will continue to develop in their role through the acquisition of further skills, knowledge and competencies, to be determined by the needs of patients, the service and the team.

This job involves a proportionate amount of time driving, you will need to be a car driver and have access to a car for work.
