Main duties of the job
As a Mountbatten Carer, you will work in accordance with individual care plans written by a named nurse to provide care and support to patients with life limiting illness, their families and carers.
You will work flexibly across all care services provided by Mountbatten which will involve working in patients own homes or other places of care.
As a Mountbatten Carer you will be required to work unsocial hours as required, including weekends/nights and bank holidays.

Job responsibilities
- To deliver direct care, treatment and emotional support for patients as required within set protocols/policies in accordance with the patients individual care plan.
- To contribute to the assessment, implementation and monitoring of patients and carers, identifying physical and/or psychological problems and liaising with the appropriate health and social care team members.
- To work in collaboration with colleagues to ensure the delivery of high quality, safe and compassionate care and support.
- To promote and maintain effective liaison with care team and other health and social agencies involved in the patients care.
- To assist in the day to day organisation of the service of carrying out duties as delegated by the Care at Home Team Leader, Clinical Nurse Specialist or Ward Manager - Inpatient Unit.
- To promote Mountbatten services through good communication with health care providers and the general public of the Isle of Wight.
- To ensure that confidentiality of information is maintained at all times.
- To provide respite care for families/carers as directed by the Senior Nurse in accordance with the patient/family/carers needs and care plan.

Experience
Essential
- Experience of providing personal care and support for patients within care in a hospital, hospice or community setting
- Experience of dealing with vulnerable adults in a care setting
Qualifications
Essential
- NVQ level 2 in health care or equivalent experience or willingness to work towards
- Good general education
Desirable
- Specialist palliative care training
Abilities
Essential
- To be an effective advocate for the patient within multi-disciplinary teams across all healthcare settings
- To effectively manage own time and workloads
- To work shift patterns on a rota basis including nights, weekend, bank holidays
- To respond to difficult situations in a sensitive manner and to deal with difficult emotional situations
Skills
Essential
- Good communication skills to explain complex and sensitive information effectively in both written and oral to both patients, families, carers and other professionals
- Good IT skills including Microsoft and email applications
- Demonstrate commitment to the patients right to be involved in decision making relating to their health care and ensuring decisions are understood by care teams

- Full UK manual driving licence
- Able to complete the manual handling requirements associated with this role
