Job summary

Community IV Nurse - 12-Month Fixed Term Contract - 37.5 hours per week

An exciting opportunity has arisen within our community IV nursing team for experienced nurses to join the team and work across our unique Integrated Acute, and Community Care service to help us make a difference to our patients. Main duties of the job

To provide effective caseload management and care to those patients over the age of 18 requiring nursing care in their own homes and other community settings working autonomously and independently.

The post holder will provide holistic care, assessment of the new patient , clinical advice, treatment and support to those people who require additional support to remain in their own homes.

The post holder will be required to work with the multidisciplinary team, gaining support and advice in the management of care. This post is subject to lone working so experience of working alone is desirable.

Qualifications

Essential

  • RN (Adult) registered with NMC
  • Specialist Practitioner Qualification (DN) OR Have any of the following modules ? Current issues in community nursing ? Leadership and innovation ? Research in health and social care
  • V100/V300
  • Any of the relevant optional modules (PACR, EOL, Long term conditions, older persons and dementia
  • Community specialist practice

Desirable

  • Evidence of leadership or relevant experience

Experience

Essential

  • Systematic approach to assessment, planning and evaluation of needs led care. ? Significant post registration experience in IV therapy. ? Able to work autonomously. ? Previous community nurse experience and lone working advantageous. ? Use of an electronic patient record system (SystmOne) ? Experience in supporting junior staff and students through supervision and opportunity to achieve learning outcomes

Desirable

  • Life experiences that have enhanced professional development. ? Experience in leading practice developments ? Experience in caseload management

Knowledge

Essential

  • Good written and oral communication skills. ? Knowledge of legislation affecting professional practice. ? Good IT skills. ? Present their application and supporting information in a clear reflective manner. ? To be able to work flexibly and sensitively. ? Be able to work well in a team as well as the ability to work on your own. ? Excellent organisational skills To be adaptable to a combination of roles which include visiting patients in their own homes and using a patient record database system to review physiological signs and symptoms. ? Competent the care of central lines ? Competent in venepuncture and cannulation
