Job Description

An exciting opportunity has arisen to join an integrated community learning disability team in Tameside and Glossop.We are seeking to recruit a highly motivated enthusiastic and forward thinking practitioner with at least two years direct experience of working with adults with a learning disability and/or autism. The post holder will be employed by Pennine Care FT and sit within the integrated leaning disability community team.

The post holder will work alongside individuals, their carers, support staff, Social Care, GMP, GMSST and the CCG to ensure best person centred outcomes are achieved. The post holder will work autonomously and collaboratively within the skilled mixed integrated team, completing highly specialist functional behaviour assessments whilst developing, implementing and reviewing specialist intervention plans. The candidate will carry a caseload and offer support to people with complex challenging behaviours referred to the service. They will offer advice and guidance to other team members in relation to positive behaviour support. A working knowledge of transforming care agenda, an interest in forensic issues and mental health will be advantageous.

The successful candidate will need to be able to demonstrate knowledge and skills in person-centred functional behaviour assessments and intervention plans. They will be a good leader and have excellent values, interpersonal and communication skills.
