Job Title : Community Mental Health Transformation Programme Lead

Salary : Band 8b (£54,764-£63,862)

Hours : 37.5 hrs per week, part time considered

Location : Sanger House

Contract : Maternity cover, fixed term 12 months

This role offers an exciting opportunity to work within the One Gloucestershire Integrated Care System (ICS) leading on the transformation of Community Mental Health Services.

Our collective vision for the programme is to radically transform our whole model of Mental Health Care and Support over the next 3 years and beyond. Increasing levels of demand and complexity in our population, coupled with new funding flows for mental health mean we have an exciting opportunity to reimagine how we deliver care across all age groups and levels of need. Our aims include a greater focus on prevention and wellbeing and delivering more integrated and personalised care closer to home:

-People with mental health problems will be enabled to manage their condition or move towards individualised recovery on their own terms, surrounded by their families, carers and social networks, and supported in their local community".

We will be working with Primary Care Networks and local partners across the county to develop multi-agency, integrated and place-based teams that support people with serious mental illness to receive their care closer to home. This ambition includes developing a new workforce with lived experience, more flexible working and continuity of care, better integration with physical health and social care and enabling greater resilience through working with third sector partners. Our aim is not just to redesign care pathways but to develop a new ethos and culture that helps transformhowwe work together with partners as one team sharing care.

The post-holder will work closely as a key member of the programme team, bringing a hands-on and practical approach and helping to define, lead and drive change. The role represents a unique opportunity to help shape and deliver an ambitious and radical programme of change, co-produced with colleagues from across the health and care landscape and with service users - for the benefit of our local populations.
