An exciting opportunity has arisen within the Urgent Community Care Team (UCCT)- a community rapid response team offering a multidisciplinary approach to hospital avoidance for the residents of Heywood, Middleton and Rochdale.
Following a five year review the UCCT has a vacancy for an enthusiastic band 7 Community Nurse to join the multidisciplinary team made up of advanced practitioners, GP’s, physiotherapists and occupational therapists, pharmacists and community nurses.
As a rapid response service in the community we have strong links with both acute and community teams including medical admissions units and intermediate care facilities. This post offers opportunities to work collaboratively with these services to promote true multidisciplinary working.
The successful candidate will be expected to lead/support on all aspects of team development and be ready to take on a new challenge to help shape an evolving service.
The UCCT are a dynamic and forward thinking team that pride themselves in going above and beyond for all. In order to support hospital avoidance you will have advanced knowledge in all areas of frail elderly care and previous experience within a multi-disciplinary team.
Regular supervision, competency training, performance reviews and Continued Personal Development are actively encouraged alongside the delivery of in-service training and support to the wider team.
