We are currently developing our multidisciplinary teams to further support service users who are discharged from hospital to the community setting; we strive to ensure these discharges are well managed, safe and timely. We are looking for Rehabilitation Support Workers to underpin these services, in close collaboration with colleagues from social care, primary care, care home providers and our clinical commissioning group (CCG) and really make a difference for these service users and their families.

We are looking for experienced workers within the health industry to support our growing number of service users in their journey to independence through the delivery of person centered rehabilitation programmes. Successful candidates will have to be comfortable working on their own in a community setting, have excellent verbal communication skills and be able to take initiative to ensure that service users and their families have the best experience being supported by us, whilst working within set guidelines.

We have full and part time hours available and can support flexible working patterns- this service operates seven days a week.
