An exciting opportunity has arisen for an enthusiastic, and highly motivated Band 5 Community Staff Nurse post to join the Burton and surrounding areas Community Nursing Team. We are an innovative service covering GP practices in the East staffs area. As a Community Staff Nurse you will deliver a high quality service to patients in their own homes, residential settings and clinics, working closely with the GP’s and multi-disciplinary teams addressing a range of care issues across the spectrum of health conditions, and preventing avoidable hospital admissions.

You will work independently in the community setting, participating in the ongoing assessment, planning, implementation and evaluation of programs of care, adhering to policies and procedures.

You must be a qualified nurse with current NMC registration, hold a full driving license and will be required to work flexibly on a rota system across a 7 days a week, including bank holidays. You should have excellent communication and interpersonal skills and the ability to organise and prioritise your workload without direct supervision.
