An opportunity has arisen for a highly motivated individual to support the Division of Integrated & Community Services.
There are various vacancies to increase Heywood, Middleton and Rochdale discharges from hospital within the Home in a Day Service and to provide a therapy service to The Discharge to Assess Beds.
The Therapist, either an Occupational Therapist or Physiotherapist will be required to provide assessment and treatment for patients across the Pennine Acute Hospital sites to support discharge into patients own home through the Discharge to assess scheme.
The Discharge to Assess Team were finalists with the HSJ Awards 2018 for Service Redesign North/Midlands and East and was recognised as an outstanding service by CQC 2020.
This new integrated service model offers the opportunity to work flexibly within a multi-disciplinary team across the Pennine Acute footprint and local Heywood, Middleton and Rochdale community, truly putting patients and service users first and offering care in the place best suited to their needs.
