We are looking for dynamic and motivated Specialist Community Practitioners (SPQDN) with experience of case management and multidisciplinary working, together with excellent communication skills, resilience and great leadership skills with the ability to motivate and support others through change in order to enable us to maintain the delivery of high quality, compassionate, proactive and safe care.

We currently have full time vacancies within our community based services and are looking for both experienced or newly qualified District Nurses to join our existing teams providing care to patients in their own homes across the Wakefield locality.

As a District Nurse within The Mid Yorkshire Hospitals NHS Trust you would gain a wealth of knowledge and experience. We aim to support you through either preceptorship or mentorship to develop in your new role and can offer additional training to further your clinical skills. We offer a fully comprehensive preceptorship and induction program and encourage all interested applicants to have an informal chat with staff in the field who can talk to you about the role.

At The Mid Yorkshire Hospitals NHS Trust we encourage life-long learning and there is an expectation that you would support pre-registration students, trainee nurse associates and District Nursing students within your role. We have excellent electronic facilities to support your learning and development and have recently developed an on-line portfolio to support your revalidation.

Working as part of the District Nursing team you will develop extensive clinical assessment and case management skills in areas such as complex wound management, End of Life care and long term conditions management, as well as case-load and people management. This will include the care of patients with diabetes, heart failure, dementia and respiratory complications.

District Nursing teams work closely with their community colleagues to maintain patients’ independence at home by utilising their excellent networks with the voluntary agencies in local communities. You will work closely with GPs and other community colleagues to plan and deliver complex packages of care to enable patients to remain in their home environment for as long as possible.
