University Hospitals of Derby and Burton NHS Foundation Trust is committed to improving End of Life care in a robust and person centred way. The End of Life Care Facilitator will work collaboratively with key stakeholders to deliver initiatives and develop programmes of education and training based on the current national guidance and policy and deliver these to all health and social care providers across all health and social care settings in order to ensure high quality care. The Facilitator will also audit and evaluate the outcomes of the educational programmes.

The role offers some flexibility and will require you to work independently and as part of a wider team. In return we offer a period of induction into the role, training where appropriate, some flexibility with working hours and a high level of job satisfaction.

This is a full time role working 37.5 hours per week.
