Job summary
An Enhanced Practice Nurse (EPN) goes beyond patient care to drive quality improvement, mentor staff, develop policy, and engage in community health initiatives. They lead on clinical projects, enhance care standards, and advocate for patient needs within the healthcare system. This role combines hands-on care with strategic contributions, making EPNs essential for advancing patient outcomes and promoting best practices.
Main duties of the job
Embedded in the team at Unity Health working as part of a multi-disciplinary team (MDT)within the PCN.
Consults with patients, their family, and the multi-professional team to undertake assessments of patient need and devise and evaluate complex care plans.
Evaluates and analyses clinical problems using their clinical knowledge, seeking out and applying relevant evidence, enhanced techniques, interventions, and equipment to make clinical decisions.
Delivers enhanced clinical care in the context of continual change, challenging environments, different models of care delivery, innovation and rapidly evolving technologies using analysis and their underpinning knowledge to manage complex interventions.
Teaches and advises patients and their families on how to manage their condition or support the multi-disciplinary team to do so.
Demonstrate initiative and are creative in finding solutions to problems.
Provide clinical supervision to the nurse team.
Maintain quality patient records and use a database to store details of referrals and care pathways for specific patients.
Implement and develop a robust clinical audit cycle and lead on research projects, implementing changes as required, including the development, and updating of practice protocols / guidelines and procedures locally.

York City Centre PCN is made up of three GP Practices, Unity Health, Dalton Terrace and Jorvik Gillygate.
Unity Health has two surgeries to the east and south of York. We have a long history, over 60 years, of providing family medicine to our local population including care homes. We also have over 30 years' experience of student health, working closely with the University of York.

Job responsibilities
Key wider job responsibilities:
Develop effective working relationship with PCN member practices.
Facilitate and support the development and delivery of innovative training and education to staff, to improve competence and confidence, in line with best practice and guidance related to area of expertise.
Participate in and or lead formal training events promoting best practice in area of expertise.
Provide professional and clinical leadership and mentorship, acting as a clinical role model on the delivery of evidence-based practice.
Advise and support on the development and delivery of work streams relating to area of expertise as part of the PCNs strategy, where appropriate.
Work proactively with key local and national stakeholders to develop more integrated care pathways and holistic models of care, through dissemination of specialist skills and knowledge.
Contribute to the delivery of local policy, procedures and protocols for the PCN to improve identification, assessment, ongoing support for patients and their family carers.
The post holder must be a Registered Healthcare Professional (e.g., HCPC or NMC registered) and will deliver services that expand and enhance the core offerings of general practice. This role requires practice at an advanced level, aligned with the competencies outlined in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.

Qualifications
Essential
- Registered Health Care Professional (eg HCPC/NMC)
- Has a post graduate qualification at level 7 or above relevant to their area of enhanced practice, for example in wound care, diabetes plus respiratory and CVD, dementia, womens health and public health and population health management.
- Has worked within primary care in services that enhance the core general practice offer.
- Working at an enhanced level of practice as described in the Primary Care and General Practice Nursing Career and Core Capabilities Framework.
- Knowledge of audit / research and experience of analysing findings to improve practice.
- Knowledge and understanding of clinical governance.
- Demonstrate the ability to work in a highly demanding environment.
- Ability to work independently and make autonomous clinical decision within own area of competence.
- Evidence of continuing professional development.
- High level communication skills, both written and verbal.
- Ability to reflect on own practice.
Desirable
- Non-Medical Prescribing Qualification.
- Relevant masters degree/180 credits at level 7 (Masters) in relevant area.
- Experience and understanding of a wide range of approaches to the management of patients with Long Term conditions.
- Awareness of current national and local agenda in health and social care, with a particular interest in Primary Care.
- Understanding and knowledge of policy developments related to the delivery of primary care services including General Practice, the GMS/PMS contract, Clinical Governance, Quality & Outcomes Framework.
