Job summary

The Community Nursing Service within North East Lincolnshire provide nursing care and support to patients, families and carers in a variety of community settings supporting patients to remain in their own homes, preventing hospital admission and facilitation of early hospital discharges.

The Community Nursing Service works closely and in partnership with Primary Care Networks, Secondary Care, Social Care, Specialist Nurses and other healthcare staff to provide a quality service tailored to meet individual needs. The service supports and assists patients to manage ill health and disability, thus allowing them to have the best possible quality of life, while maintaining dignity and respect.

Community Nursing

Pilgrim Primary Care Centre, Immingham

Hours: 37.5 hrs a week - you will be required to work a rota pattern which includes weekends and Bank Holidays

Salary Range 3 £23,807 fixed salary point

You will work on your own in the community, under direction of the team, to provide a range of care needs to our clients, such as wound dressings, eye care, pressure area care, venepuncture, monitoring of blood pressure, weight and blood sugar as well as palliative and terminal care. You will also provide emotional support to patients, carers and family members therefore must have an empathetic and sympathetic approach with effective communication skills. Most importantly you must be able to demonstrate your commitment to the values of Care Plus Group.

You must hold a relevant qualification, i.e. QCF Level 3 in Health and Social Care. A Level 2 qualification in Numeracy and Literacy is also essential or an ability to demonstrate an equivalent level of competence. You will have accurate and proven record keeping and IT skills.

In return you will have access to further training and development with supervision and support.

You must be able to transport yourself and equipment across locations within North East Lincolnshire to deadlines and you will be required to work evenings, weekends and bank holidays on a rota basis showing a flexible approach to working arrangements.

Qualifications
