The Black Country Healthcare NHS Foundation Trust is seeking an enthusiastic and motivated practitioner to work autonomously as a Health Visitor committed to achieving positive health outcomes for pre-school children and families and to ensure the safeguarding of children and vulnerable adults.

We welcome experienced and newly qualified health visitors to apply to join us in utilising their Specialist Public Health Practitioner Nursing/Health Visiting knowledge and skills to play a pivotal role working within the Dudley Borough. The post holder will receive a comprehensive induction programme and for newly qualified staff, a preceptorship framework is in place.

Successful applicants will work as part of a diverse Health Visiting team led by supportive Health Visitor Team Leaders and teams including Practice Teachers, Nursery Nurses and administrative staff. The post holder will carry continuing responsibilities for caseload management and the assessment of care needs as part of the service offer for health visiting to include Community, Universal, Universal Plus and Universal Partnership Plus services. This will involve being responsible for the delivery of the Healthy Child Programme and the development, implementation and evaluation of programmes of care which are sensitive, relevant and responsive to the diverse needs of individual and targeted patients/clients, families, groups and communities. The post holder will also contribute to the Public Health Agenda to meet Public Health targets and address health inequalities.
