An exciting opportunity has arisen for a suitably qualified and experienced individual to join the South Tyneside Community Palliative Care Team in the post of Hospital and Community Specialist Palliative Care Nurse to work 30 hours per week over 4 days.

The Post holder will manage an active case load across Hospital and Community site, and be expected to work in both the acute and community setting as required.

The post holder will be part of a Specialist Palliative Care Team which incorporates community and hospital consultants and a multi- disciplinary team.

The specialist palliative care nurses provide professional advice and support to the in and out of hours palliative care teams.

The successful candidate will be educated to post graduate level, and have suitable community and hospital/hospice experience within the field of specialist palliative and end of life care. Excellent communication and leadership skills will be an expectation of the post, as well as a strong proven commitment to service development.

The post holder will be expected to be able to work collaboratively as part of a team, and build effective relationships with colleagues, whilst also being able to work on their own initiative. The post holder will also need to be able to priorities and work to deadlines without compromising quality of care

Appropriate support and development for the successful applicant will be negotiated on an individual basis
