The Isle of Wight NHS Trust has an exciting opportunity for a Registered Nurse to join the Community Rehabilitation Single Point of Access team. We are looking for an individual who will be a key member within the Community Rehab Service, committed to providing the highest standards of support and care.

 

Currently based in the Single Point of Access team you will be responsible for providing high standard rehabilitation nursing support to the Community Rehabilitation Service. The post holder will work within the acute hospital setting and rehabilitation to identify through assessment, appropriate rehab candidates and non-rehab candidates to follow the correct pathway for discharge from hospital. This will follow National guidance.

 

The post holder will be NMC registered and have relevant previous UK NHS experience. The post requires comprehensive experience developing and supervising more junior members of the team.

Community Rehabilitation is changing rapidly –We currently provide all aspects of nursing care for rehab candidates in the community. We are working towards a locality model of rehab provision. The team includes Nurses, Occupational Therapists, Physiotherapists., Speech & Language Therapists, Social Workers, Associate Practitioners and rehabilitation assistants. We work closely with our Nursing Home providers and GP’s.

Support and training will be available to ensure a safe transition to working in the community rehabilitation and Acute Hospital setting.
