An exciting opportunity has arisen for an experienced Nurse Commissioner within NHS Continuing Healthcare to undertake a leadership and management role within the CCG’s Individualised Care Department.
The department is responsible for NHS Continuing Care, NHS-funded Nursing Care, Complex Care and Children’s Continuing Care and the Personal Health Budgets agenda. The successful candidate will have knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care (2018); National Framework for Children and Young People’s Continuing Care (2016), and current National Guidance and Policies relating to these areas of work, including Personal Health Budgets.
The post holder will oversee and develop various functions relating to the Commissioning of Individualised Care and will support the Assistant Director for Individualised Care to ensure CCG compliance with the requirements of the above Frameworks, Guidance and Policies. The post involves frequent and ongoing engagement with a range of individuals including health and social care professionals, care providers and patients and their representatives and it is therefore essential that the successful candidate has highly effective verbal and written communication skills.
The post holder will be innovative and dynamic in their approach. They will require the ability to deal with a range of highly complex and challenging situations on a frequent basis and to provide support, guidance, and supervision to team members.
The successful applicant will have the ability to respond to the challenges of the post and be adaptable, flexible, and knowledgeable about the changes and demands within the health and social care economy.
