Our highly regarded Collaborative Care Service is a busy team serving the patient populations in South Northamptonshire. The service operates from 0900-1800hrs, Monday to Friday. The team works closely with colleagues in both primary / community settings to provide assessment of acute problems to help patients remain in their own home environment.
The role will require you to provide timely, high quality, person centred clinical care to patients addressing their current needs and their long term self-management. Acting as a self-directed practitioner providing and evaluating the highest standard of evidence based clinical assessment to patients within their home environment.

Successful applicants will:
- have a minimum of three years post registration demonstrable experience of working as an independent practitioner
- experience of working in a community setting
