An exciting opportunity has arisen for a Band 6 Lead Mental Health Nurse to join the Older Adult In-reach and Home Treatment Team.

The primary goal of the service is to support people within their own homes, who are experiencing a breakdown of their mental health and to prevent admissions to hospital where appropriate.

The successful applicant will work primarily within the In reach part of our service. You will work closely with an already established In reach team, and also the MDT team on ward 1, and support early discharges and subsequent home treatment.

The successful applicant will also have close links with the Community Mental Health Teams, GPs and social care.

There is an expectation that as a Lead Nurse, you will be able to make complex decisions, and formulate detailed assessments and person-centred care plans to assist in the prevention of hospital re admissions where appropriate.

We cover Derby City and the the surrounding areas of Derbyshire.

Experience of working with older adults with a functional, mental health illness is essential.

There is the opportunity to work within the Home treatment side of the service also.

We are also co-located with our established, Dementia Rapid Response Team Team.

Professional development and further learning is encouraged and you will receive robust clinical and managerial supervision, along with wider support and training in-house within the team.

You will also be expected to provide supervision to team members and mentor students.

You will be working Monday-Friday, 9am-5pm.

The team covers 365 days a year.
