We are currently looking for an enthusiastic and efficient Pathway Facilitator where you will be the main point of contact for the MDT for discharge planning from the specialist palliative care unit.

Supporting throughout the patients journey, you will work as part of the MDT and closely with external colleagues such as Continuing healthcare, the CCGs and Social Services. Supporting a case management style of working, you will ensure that all patients who require support to leave hospital are managed and facilitated.

You need to have experience in the health and social care settings and have excellent communication skills. You will be able to work autonomously with your own initiative and have the ability to adapt to change while managing and prioritising a heavy workload, in a fast-developing environment. You will ensure high quality holistic and individualised care to patients and their families.
