The Clinical Pharmacy in General Practice program aims to transform care by integrating pharmacy roles within primary care to deliver patient-centered, coordinated services. This model aligns with national policies, such as the Five Year Forward View and GP Forward View, to leverage pharmacy expertise in helping patients stay safe, healthy, and out of hospitals, while reducing demands on general practices. Pharmacy technicians, while distinct from clinical pharmacists as they do not prescribe or make clinical decisions, play a crucial role in the primary care network (PCN) team by supporting medication management and optimizing medicine use under supervision. Their responsibilities include clinical, technical, and administrative tasks to enhance safe, cost-effective prescribing, improve repeat prescription processes, and minimize waste. Additionally, they promote medicines optimization, support adherence to NICE guidelines, and help the PCN meet national standards and therapeutic advancements, ultimately enhancing patient care quality. Key Responsibilities: Undertake patient facing and patient supporting roles to ensure effective medicines use, through shared decision-making conversations with patients. Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively. Provide expertise to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities. Manage shared care protocols and liaise with Clinical Pharmacists for more complex patients. Support the PCN multi-disciplinary team to ensure efficient medicines optimisation processes are being followed. Essential qualifications and experience: Experience of working as a qualified, registered pharmacy technician in primary care, community, or hospital pharmacy. Registered as a pharmacy professional with the General Pharmaceutical Council (GPhC) Meets GPhC qualification and training standards required for pharmacy technician registration. Enrolled in, pursuing, or qualified from an approved training program, such as the Primary Care Pharmacy Educational Pathway (PCPEP) or Medicines Optimisation in Care Homes (MOCH) Demonstrates evidence of ongoing professional development (CPD) Skilled in analyzing and interpreting prescribing data.
