Cannock Community intervention Service requires a Band 6 Physiotherapist.


An exciting opportunity has arisen to work as a Physiotherapist within a multi-disciplinary team within the Cannock Chase District. This is a permanent, full time,37.50 hour post that will be based in Norton Canes Medical Centre, but will involve travel across Cannock and surrounding areas.

You will be joining a multi-skilled team of specialists including, other Physiotherapists, Occupational Therapists, Assistant Practitioners, Advanced Nurse Practitioners, and Nursing Staff.

You will be responsible to the Operational Lead and will have support from other senior colleagues within the wider integrated community team. The service currently covers 5 days a week from Monday to Friday. Your role as part of this service will be to provide assessment and ongoing therapy management plans for adult intermediate care patients, step up or step down, and discharge to assess patients in a variety of settings.

You will be working in partnership with colleagues within our integrated service and a good knowledge base of the services and equipment now available to support independence in the home environment would be desirable.
