Job summary

The post holder will work alongside the Practices team of GP's & Clinical Staff to provide a high level of care to the 20,000 patients registered with the Practice.

We have an opportunity for someone passionate about patient care to join our friendly and supportive practice. We are looking for a qualified Practice Nurse to support our GP's and wider clinical team. To include Respiratory knowledge.

Main duties of the job

Conduct consultation appointments with patients, both in person and over the phone.

Perform Initial assessment and examination of patients: taking medical histories and completing physical examinations.

Request and interpret diagnostic tests and laboratory investigations where necessary.

Diagnose medical problems and conditions. Develop treatment and management plans. Confidently explain these to the patient concerned.

Provide direct care to patients using established clinical guidelines for both routine and urgent complaints.

Refer patients to specialists and other health professionals where appropriate.

Educate patients in preventative health care.

Complete home/care home visits to patients, when required.

Work within the remit of a Practice Nurse and know the boundaries of the role. Escalate concerns and difficult cases appropriately.

About us

MyHealth are a well-established, innovative, high achieving, training and research active practice. We have four branch surgeries located in picturesque, semi-rural area just north of York. With agrowing list of nearly 20,000 patients, we continue to develop our service and our wide and varied team.

We currently have 14 GPs who work closely with a strong team of dedicated allied health professionals, including Paramedics, Physiotherapists, Advanced Nurse Practitioners Practice & Treatment Room Nurses and Pharmacists. All are supported by a team of well-trained and committed administrators. We try to be as paper-light as possible and use SystemOne as our clinical system.

Job responsibilities

POST: Practice Nurse

LOCATION: Any My Health Site

RESPONSIBLE TO: Nurse Manager

JOB PURPOSE:

To be responsible for the delivery of nursing services, working as part of the practice multidisciplinary team, delivering care within their scope of practice to the entitled patient population. The nurse practitioner will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well women and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

Responsible and accountable to the appropriate Nurse Manager, working within a team of Nurses and Health Care Assistants.

MAIN ACCOUNTABILITIES

The following are the core responsibilities of the nurse practitioner. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

Develop, implement and embed health promotion and well-being programmes

Provide a triage service, actively managing or referring patients or treating patients within own scope of practice

Support the clinical team in the development of individual treatment plans for chronic disease patients

Identify, manage and support patients as risk of developing long-term conditions, preventing adverse effects on the patients health

Provide routine nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF

Provide wound care (ulcer /Doppler etc.) to patients

Provide travel medicine services

Provide Respiratory care

Request pathology services as necessary, processing and interpreting results as required

Refer patients directly to other services as appropriate

Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary / specialist care as required

Maintain accurate clinical records in conjunction with extant legislation

Ensure read codes are used effectively

Maintain chronic disease registers

Develop, implement and embed well woman clinics

Develop, implement and embed well man clinics

Chaperone patients where necessary

Assist GPs with minor surgery when required

Prioritise health issues and intervene appropriately

Support the team in dealing with clinical emergencies

Recognise, assess and refer patients presenting with mental health needs

Recognise, assess and refer patients with community health needs, developing strategies for effective care pathways

Implement vaccination programmes for adults and children

Liaise with external services / agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual)

Support the clinical team with all safeguarding matters, in accordance with local and national policies

Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

Deliver opportunistic health promotion where appropriate

In addition to the primary responsibilities, the nurse practitioner may be requested to:

If an independent prescriber, prescribe in a safe an effective manner, in accordance with PGD and extant legislation

Act as the audit lead, effectively utilising the audit cycle

Monitor and ensure the safe storage, rotation and disposal of medicaments

Contribute to the implementation of an effective training programme for staff and students within the practice

Provide clinical supervision for the nursing team

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the practice

Continually review clinical practices, responding to national policies and initiatives where appropriate

Participate in the review of significant and near-miss events applying a structured approach i.e. root cause analysis (RCA)

Maintain professional registration and adhere to re validation requirements

This Job Description will be reviewed each year as part of the annual appraisal process, to ensure that it reflects the responsibilities of the post. No changes will be made without full consultation with the post holder.
