Make a difference to patient care, grow your clinical skills and manage your own patients on the Fylde Coast as part of a Community Frailty Service.

We are looking for an enthusiastic, highly motivated pharmacist to deliver high class quality clinical care to some of our most vulnerable frail patients. You will play a key role in leading on medicines optimisation and provide expert clinical advice on the management of long term conditions to patients and colleagues.

The Extensive Care Team consists of a Consultant Geriatrician, GP’s, Advanced Practitioners (and trainees), Clinical Care Coordinators, a Pharmacist, Pharmacy technicians and Assistant Practitioners.

You will be working alongside a Consultant and GP’s and will benefit from their clinical leadership and experience whilst working within the service.

You will be part of a core pharmacy team including a Pharmacist and a team of technicians. This post offers the exciting opportunity for the individual to influence service provision, and support patients with complex needs by providing expert pharmaceutical and clinical advice.

The Extensive Care Service provides a comprehensive assessment of the individual’s physical, mental health and social care needs resulting in the development of a single shared personalised care plan with patient centred goals and measurable outcomes. Patients and carers will be supported to manage their own conditions, and provided with tools to recognise deterioration triggers.

You will be confident in delivering medication reviews and assessments in a clinic setting, and if required within a patient’s own home. You will also hold a Prescribing qualification

In addition to well-developed communication and leadership skills you will have previous experience of working within a clinical environment and will have completed a post-graduate diploma in clinical pharmacy, or have equivalent post registration experience. Evidence of multi-disciplinary teamwork would be advantageous.
