Job summary

We have a vacancy for 37.5 hours per week.

We are looking for a person-centred Social Worker to join our award-winning Urgent Care team. This is an exciting time to join the Community Crisis Response Team as investment allows expansion of roles in admission avoidance and the development of exciting new initiatives. All roles are valued members of this highly integrated team and you will gain great satisfaction in using your skills, in collaboration with trusted health and social care colleagues, to work with patients during their health crisis.

The team includes Physiotherapists, Occupational Therapists, General Nurses, Mental Health Nurses, Social Workers, Community Care Workers, Paramedics, support workers and coordinators. In addition, there are opportunities for working with Acute Response Practitioners within the team to expand your understanding of physical assessment and clinical reasoning of frail elderly patients in primary care.

CCRT promotes a culture of learning and improvement, and as part of our team you will have the opportunity to gain desirable interdisciplinary skills whilst maintaining your social care identity. Staff are supported with a regular training programme to meet team competencies, as well as encouragement to attend relevant professional courses.

Main duties of the job

  • Manages a day to day case load and will be required to assess and support people to live independent lives in the context of intermediate care criteria and statutory duties in order to enable vulnerable people to manage their own lives.
  • The post holder reports to the Advanced Practitioner via annual performance appraisals and six monthly reviews, 1:1 meetings where case work can be discussed as necessary and regular professional supervision.
  • Responsibility for holistic and partnership assessments of service users and their carers who have complex needs, utilising Social Work theories, personalised approaches models and methods to identify strengths and needs to inform appropriate interventions.
  • Assess the urgency of a request for action and identify the need for legal and procedural intervention.
  • Complete assessments through detailed conversations with all groups of clients that may include those with mental health illnesses, learning disabilities, substance and alcohol misuse, sensory impairment, acquired brain injuries and a wide range of physical disabilities.
  • Assess the outcome of any intervention against need, professional standards and best practice under the Care Act 2014.
  • Assess eligibility and negotiate immediacy of intervention against resources, risk and need. The post holder will need to have an awareness of budgetary constraints.

To be legally literate with the Care Act (2014), the Mental Capacity Act (2005) and Human Rights Act (1998). To have a good understanding of the Mental Health Act (1983) and its subsequent amendments and the Children Act (1989).

Preparing legal documents for court and representing the organisation in a court setting.

Maintain contact with people who use our services either face-to-face, telephone or assistive technology such as video links and conference calls.

Design, implement and review care and support plans, which may involve the input of other professionals /disciplines.

Work to a number of protocols/procedures such as lone working, risk management and health and safety.

Complete carers assessments and seek feedback from people who use the service.

Learning some basic clinical skills to enable the undertaking of holistic assessments and liaison with the multi-disciplinary team.

Take an asset-based approach, including signposting to universal services, advice and information and considering the persons links and integration with their community and services such as advocacy.

Duty of candour and supporting people to make complaints if they wish too.

Maintain positive working relationships and communicate clearly with team colleagues, line manager, other professionals, people who use our service and their families/carers which may include highly sensitive issues.

Listen to and work in partnership with the people who use our services and families and carers.

Maintain professional conduct and communicate clearly when faced with competing demands and in situations that are potentially highly emotive, such as being challenged by families or other professionals.

Present evidence-based decisions and judgements about potentially complex/ ambiguous situations in a manner fit for the audience. This includes writing court reports, explaining legal requirements to people and their families and projecting social work values in a multi-disciplinary setting.

Working closely with the integrated locality teams, Ageing well MDT, GP and voluntary sector.

Act as a patient advocate as required ensuring individual needs, preferences and choices are delivered by the Service.

Person Specification

Qualifications

Essential

  • You must hold a qualification in Social Work, such as postgraduate diploma, bachelors degree or masters level.
  • You must have a valid Social Work England professional registration.

Desirable

  • Any postgraduate qualifications relevant to social work, such as Graduate Certificate in Practice Education or Approved Mental Health Professional

Experience

Essential

  • Proven relevant experience of working in the health and social care sector.

Desirable

  • Experience of working with other professionals in an integrated service

Knowledge and skills

Essential

  • Knowledge of social work theories and models, the strengths and limitations and how they can be applied to practice.
