Cannock Community intervention Service requires a Band 4 Assistant therapy Practitioner.
An exciting opportunity has arisen to work as a Therapy Assistant Practitioner within a multi-disciplinary team within the Cannock Chase District. This is a permanent Part time 22.50 hour post that will be based in Norton Canes Medical Centre, but will involve car travel across Cannock and surrounding areas.
You will be joining a multi-skilled team of specialists including, Physiotherapists, Occupational Therapists, other Assistant Practitioners, Advanced Nurse Practitioners, and Nursing Staff.
You will be responsible to the Operational Lead but will have support from other senior colleagues within the wider integrated community team. The service currently covers 7 days a week. Your role as part of this service will be to provide support and ongoing therapy working from therapy plans set by senior colleagues. You would support OT and physio on a daily basis but also be expected to work independently visiting patients such as adult intermediate care patients, step up or step down, and discharge to assess patients in a variety of settings in the community. Your main aim will be to promote independence to the patients in their community setting.
You will be working in partnership with colleagues within our integrated service and a good knowledge base of the services and equipment now available to support independence in the home environment would be desirable.
