Do you want to help shape service delivery in relation to hospital discharge working with County Durham and Darlington NHS Foundation Trust?

This is an exciting opportunity for an experienced leader to play a key role in delivering high-quality hospital discharge services across County Durham. Working in partnership with health and social care colleagues, you will help ensure people leave hospital safely, at the right time and with the support they need to maximise their independence and wellbeing.

We are looking for a creative, forward-thinking and solution-focused individual to join us as a Hospital Discharge Team Manager (Countywide). You will lead a dedicated team, drive service development and support innovative approaches to hospital discharge. This is an excellent opportunity to influence practice, strengthen partnership working and deliver positive outcomes for residents through strengths-based, person-centred approaches.

WHAT IS INVOLVED?

You will support and oversee a countywide cohort of social work staff and Assistant Team Managers in relation to hospital discharge in County Durham.

You will be expected to have a good understanding of discharge pathways, staff management, financial budget management, Transfer of Care Hubs and the Home First Model. You will also provide day-to-day supervisory support to administrative staff, promoting productivity, maintaining service standards, and supporting professional development.

WHAT WILL I NEED?

  • Qualification in Social work at progressed level
  • Social Work registration
