Job summary
We are looking for a part time Advanced Nurse Practioner to join Partners in Health, part of G DOC Ltd, working within a supportive, innovative primary care group.
- 30.5 hours per week
- Location: Partners in Health, Pavilion Stroud Road & St James Family Doctors
- £25.48 per hour
Main duties of the job
The ACP/AP will work autonomously within their scope of practice to assess, diagnose, treat, and manage patients presenting to the practice. They will provide expert clinical care, work alongside the multidisciplinary team, and contribute to service development to meet the needs of the practice population.

Job responsibilities
Duties specific to role
Within competencies and training:
- Undertake comprehensive assessment, diagnosis, and treatment of patients with undifferentiated and undiagnosed problems, both acute and chronic.
- Prescribe and review medication in accordance with national and local prescribing guidelines.
- Order and interpret investigations to inform clinical decision-making.
- Manage patients across the lifespan, including paediatric, adult, and elderly populations.
- Identify and manage long-term conditions, health promotion, and disease prevention strategies.
- Recognise and respond to medical emergencies and deteriorating patients.
- Work effectively across the 4 pillars of advanced practice: clinical, leadership, research and education
Professional
- Maintain professional registration with the appropriate regulatory body (NMC/HCPC/GPhC).
- Work within national ACP/AP frameworks, scope of practice, and local protocols.
- Engage in clinical supervision and maintain an up-to-date professional portfolio.
- Participate in continuing professional development and annual appraisal.
- Contribute to quality improvement, audit, and clinical governance activities.
- Support the development of pathways, protocols, and practice policies.
- Mentor, supervise where applicable, and support junior clinicians, nurses and trainees within the practice.
- Communicate effectively with patients, carers, and other healthcare professionals.
- Document consultations clearly and accurately in the patient record
- Liaise with community services, secondary care, and social care teams to ensure continuity of care.
