Job summary

  • We are looking for an Advanced Nurse Practitioner (ANP) to join our team working 16 hours per week. Providing high-quality, patient-centred clinical care within a GP practice setting. Working autonomously and as part of a multidisciplinary team, the ANP will assess, diagnose, treat, and manage patients presenting with both acute and chronic conditions, ensuring safe, effective, and evidence-based care.

Main duties of the job

  • Clinical Practice

Assess, diagnose, and treat patients presenting with acute and chronic conditions

Undertake clinical examinations and interpret findings to formulate management plans

Prescribe medications in accordance with independent prescribing qualification (if applicable)

Provide advice on health promotion, disease prevention, and self-management

Manage patients with long-term conditions (e.g., diabetes, asthma, COPD)

Carry out home visits where appropriate

Identify and appropriately refer complex or high-risk cases

Ensure equipment, instruments, and clinical environment meet required safety standards

Deliver safe, effective, and evidence-based clinical care

Work within agreed protocols, policies, and clinical guidelines

Refer patients to other healthcare professionals as appropriate

Job responsibilities

Clinical Practice

Assess, diagnose, and treat patients presenting with acute and chronic conditions

Participate in practice meetings and contribute to service development

Clinical Governance & Quality

Maintain accurate and contemporaneous clinical records

Participate in audit, including minor surgery outcomes and infection control audits

Adhere to CQC standards, infection prevention protocols, and safeguarding procedures

Ensure compliance with professional codes of practice (e.g., NMC)

Communication

Communicate effectively with patients, relatives, and the healthcare team

Provide clear explanations regarding procedures, risks, and aftercare

Obtain informed consent prior to all minor surgical procedures

Manage sensitive or complex consultations with empathy and professionalism

Professional Development

Maintain up-to-date clinical knowledge and procedural competencies

Undertake training relevant to minor surgery and advanced practice

Participate in continuing professional development (CPD) and revalidation

Keep abreast of changes in primary care, clinical guidelines, and surgical techniques

Person Specification

Qualifications

Essential

  • Registered Nurse with active NMC
  • Master's level qualification in Advanced Clinical Practice
  • Independent/Supplementary Prescriber qualification (V300)
  • Post-registration experience in primary care
  • Experience assessing, diagnosing, treating, and managing patients autonomously.
  • Experience managing both acute and long-term conditions.

Desirable

  • Systmone experience
