Job summary

We are looking to recruit an experienced Advanced Practitioner to support the clinical team in our busy GP Surgery in Redcar.

This is an existing opportunity for a highly skilled clinician to take on a role with autonomy, responsibility, and scope to influence patient care.

Main duties of the job

This is an autonomous clinical role combining patient assessment, diagnosis, prescribing, and clinical leadership, with strong expectations around service improvement, team support, and safe, evidence based practice.

Job responsibilities

Key responsibilities:

Clinical assessment of patients with acute, urgent and long term conditions providing subject matter advice.

Independent diagnosis and treatment within scope of professional practice.

Prescribing and management of complex medication regimes.

Risk management and safe clinical decision making.

Clinical leadership, supporting, supervising and mentoring members of the nursing team.

Implement and evaluate individual specialised treatment plans for chronic disease patients.

Domiciliary care - seeing patients in their own homes who are too ill to attend surgery and/or performing a ward round/virtual ward round at our registered Nursing Homes.

Request and collect pathology samples, upon receipt, process and interpret pathology and other tests as requested.

Make referrals to other healthcare professionals or services when clinically indicated

Recognise, assess and refer patient with mental health needs.

Contribute towards Practice targets (QOF, etc), complying with local and regional guidance

Liaise with external services/agencies to ensure the patient is supported.

Deliver opportunistic health promotion where appropriate.

To communicate at all levels within the team ensuring an effective service is delivered.

Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of the individual) .

To maintain accurate, up-to-date healthcare records appropriate to the consultation.

Ensure evidenced-based care is delivered to the highest standards.

Enhance own performance through continuous professional development, imparting own knowledge and behaviours to meet the needs of the service.

To participate in the audit process, evaluation and implementing plans and Practice change in order to meet patient need.

Contribute positively to the effectiveness and efficiency of the team and work colleagues.

Attend and participate in Practice meetings as required.

Ensure clinical Practice is safe and effective and remains within boundaries of competence, and to acknowledge limitations.

To enable clinicians to safely travel to visit patients in their own home; they are obliged to ensure that your vehicle is in good working order and covered by business class car insurance (any additional premium will be paid by the practice upon receipt of a copy of the vehicle insurance cost).

Contribute to service development, QI projects and practice protocols.

Collaborative MDT working with GPs, Pharmacists, and other heath care professionals

Upon request, a percentage of the Advanced Practitioner's time may be spent on personal and practice development activities including audit, research, team development and clinical governance.

Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels.

Wider responsibilities:

In addition to the primary responsibilities, the Advanced Practitioner has the following wider responsibilities:

Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance.

Support the IPC lead for the practice, ensuring compliance

Participate in local initiatives to enhance service delivery and patient care

Support and participate in shared learning within the organisation.

Develop an area of specialist interest, taking the lead within the organisation.

Continually review clinical practices, responding to national policies and initiatives where appropriate.

Drive the development of nursing services within the organisation, liaising with external agencies and professional organisations as required.

Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation.

  • Knowledge and Education:
  • Qualified Paramedic or RGN with Independent Prescribing Qualification and clinical skills
  • Current NMC registration/HCPC registration
  • Evidence of working at an enhanced level

  • Skills/Abilities:
  • An excellent standard of written and verbal english
  • Excellent interpersonal, influencing negotiation and organisation skills
  • Ability to make autonomous decisions
  • Ability to assess, minimise and manage risks and defuse stressful situations, ability to stay calm
  • Understanding the importance of evidence-based practice
  • Ability to work within own scope of Practice and understanding when to refer to GPs
  • Competent use of IT systems, Excel, Word and Outlook.
  • Work independently using policies & protocols
  • Good leadership skills, teaching and developing others
  • Experience:
  • Experience of working in an urgent care, emergency care or primary care setting.
  • Ability to work within a team to provide the best outcome for the patient.
  • An ability to use own judgement, resourcefulness, common sense and local knowledge, to respond to enquiries and requests while adhering to Practice limitations.
  • Recognise and work within own competence and professional code of conduct as regulated by NMC or HCPC
  • An appreciation of the new NHS landscape including the relationship between individual organisations, PCNs and the commissioners
