Job summary

We are looking to recruit two proactive and compassionate Assertive Outreach Support Workers to join our Community Mental Health Hubs (CMHH). One role will be based across Queen's Park & Paddington hub (Woodfield Road), and one within the South Hub (190 Vauxhall Bridge Road).

You will support individuals with complex mental health needs who are at risk of disengagement, providing practical, person-centred care to promote engagement, independence and recovery.

Main duties of the job

This is a varied and rewarding role working directly with individuals with complex mental health needs who may find it difficult to engage with services. Using an assertive outreach approach, you will build therapeutic relationships and work flexibly in the community to support engagement and recovery.

You will provide practical support to help individuals develop daily routines, maintain independence, and engage in meaningful activities such as self-care, social connection, education, or employment. This includes accompanying individuals to appointments, supporting access to services, and helping overcome barriers to engagement.

Working alongside clinicians, you will contribute to care planning, risk management, and delivery of time-limited interventions. You will support early identification of deterioration, contribute to de-escalation, and help reduce the need for hospital admission.

The role involves working with carers and partner agencies, including health, social care, and voluntary services, to promote continuity of care and social inclusion.

The post holder will work as part of the Community Mental Health Hub (CMHH), supporting the delivery of high-quality, person-centred care to adults with severe and complex mental health needs who may be at risk of disengagement from services.

Key responsibilities include:

  • Supporting the delivery of recovery-focused, trauma-informed care in line with agreed care plans, under the supervision of qualified staff
  • Providing practical, structured and time-limited interventions to promote engagement, stability and independence
  • Working proactively within community settings, including home visits, outreach and community-based interventions, to engage service users
  • Supporting individuals to develop and maintain daily routines, life skills and participation in meaningful activity
  • Assisting with care planning, risk assessment and risk management, including identifying early signs of deterioration
  • Promoting access to services, including health, social care and community resources, and supporting attendance at appointments
  • Building therapeutic relationships to support engagement and positive behavioural change
  • Working collaboratively within the multidisciplinary team and with partner agencies, including housing and voluntary sector services
  • Acting as a link worker with identified local hostels to support engagement and continuity of care
  • Maintaining accurate and timely clinical records in line with Trust policy
  • Contributing to service development, quality improvement and team working

Person Specification

Qualifications

Essential

  • GCSE English (A * - C) / National Certificate in Literacy / ESOL Level 2 or equivalent
  • NVQ Level 3 in Health & Social Care or equivalent.
  • Evidence of other healthcare training

Desirable

  • Relevant training in health, social care, or housing support.
  • Additional training (e.g. first aid, manual handling, health & safety).

Experience

Essential

  • Experience working with people with mental health needs or other vulnerabilities.
  • Experience working with people who may be reluctant to engage.

Desirable

  • Relevant training in related psychosocial interventions.

OTHER

Essential

  • Willingness to undertake further training and development. g and development.
  • Able to work flexibly.
  • Demonstrates understanding of equality, diversity, and inclusion.
