Job summary

Apollo PCN, based in North Warwickshire, is looking for an enthusiastic and proactive Care Coordinator to join their patient focused, forward thinking team. The successful candidate will work alongside our clinical and non-clinical teams to offer a friendly, caring and supportive experience for patients.

You will be expected to work with several practices and will report to the Clinical Director/PCN Manager

We would also consider less than full time working

Main duties of the job

Use EMIS and Docman to manage the PCN related projects

working closely with primary care teams as well as community organisations and other healthcare professionals. Excellent communications skills are paramount with patients, families and healthcare professionals to enhance engagement and quality of life and wellbeing.

Participate in PCN projects for long term conditions, frailty and others.

There may be specific project work to support our Practices contractual obligations.

Holding care coordinator phone and taking patient queries.

Demonstrate a flexible attitude and be prepared to carry out other administrative and PA duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner;

Job responsibilities

Job Summary

Apollo PCN, based in North Warwickshire, is looking for an enthusiastic and proactive Care Coordinator to join their patient focused, forward thinking team. The successful candidate will work alongside our clinical and non-clinical teams to offer a friendly, caring and supportive experience for patients.

You will be expected to work with up to five practices and will report to the Clinical Director.

Supporting projects, managing care coordinator phone, managing patient queries.

Work Location

Mainly based at Hartshill Health Centre, but some travel expected to other Practices within the Nuneaton and Coleshill areas.

Main Duties

Patient focus and support is key to this job role, to provide time and expertise to help guide patients through the healthcare and social care system, to make connections with services and teams, giving opportunity to the patient to be heard and understood, whilst also advocating for them, helping individuals understand their rights and options.

Support the PCN practices to deliver on, QOF, KPIs and locally commissioned enhanced servicesExcellent communications skills are paramount with patients, families and healthcare professionals to enhance engagement and quality of life and wellbeing.

There may be specific project work to support our Practices contractual obligations.

Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner;

Identify opportunities and gaps in the service and provide feedback to continually improve the service and contribute to business planning;

Person Specification

Essential

EMIS Knowledge

Commitment to patient focused care

Professionalism and dedicated to personal development

NVQ Level 2 or 3 in Health and Social Care or equivalent.

Experience

Essential

Experience in supporting individuals, their families and carers

Good IT understanding, using IT systems and packages including EMIS -Experience in collecting data, project support and excellent knowledge of EMIS required

Excellent communication and record keeping skills regarding Rota management, leave management admin part, supporting day to day running admin tasks

Good understanding and respect of individual lifestyles and diverse backgrounds.

Excellent time keeping skills and ability to prioritise and complete tasks.

Works well under pressure

Good team relationships

Desirable

Good understanding of policies, procedures such as confidentiality, safeguarding, information governance and health and safety.

DBS

It will be necessary for a submission for Disclose to be made to the Disclosure and Barring Service (DBS) to check for any previous criminal convictions.

UK Registration

Applicants must have a current UK professional Registration.
