Job summary
As a Care home Care Co-ordinator you will work as part of the Enhanced Health in Care Homes team in Darlington. You will play an administrative role in co-ordinating care planning and support for residents in CQC registered care homes in the town. You will have responsibility for arranging meetings of the Multi-disciplinary teams supporting residents in care homes. You will liaise closely with GPs, Practice staff, community nursing staff and other NHS professionals, acting as part of a wider multi-agency integrated team.
This role involves working with residents and teams in care homes to ensure that the care is aligned to residents needs. It is to ensure equitable care. This is an interesting job opportunity for an experienced administrator to make a difference to care home residents.
In this position, you will be employed by Primary Healthcare Darlington on behalf of Darlington PCN in a role funded through the Additional Roles Reimbursement Scheme - ARRS and the General Practice Network Contract Directed Enhanced Service 2026/27. As part of your role, you will work closely with the PCN and the General Practices across Darlington
Main duties of the job
You will be organised, dedicated, people focused and enjoy working with a wide range of people. You will have good written and verbal communication skills and strong organisational and time management skills. You will have a strong administrative background preferably gained in a healthcare or care home setting but this is not essential.

The post holder will be required to maintain constructive relationships with a broad range of internal and external stakeholders within and beyond the organisation. Key relationships will include:

Responsibilities
To liaise with care home managers to identify those residents who would benefit from having a review.
To organize regular meetings of Multi-disciplinary teams (MDTs) supporting each care home and the EHICH clinical team.
To ensure all residents receive a comprehensive geriatric assessment
To ensure care plans are completed for each resident
To foster good communication between care home managers and all members of the MDT
To maintain and develop engagement with GPs, practice staff and community care staff.
To liaise with multi agencies to coordinate pathways of care for residents
To help residents manage their needs, answering their queries and supporting them to access primary care services
To ensure residents have good quality information to enable them to make choices about their care
To collect data on residents for recognised outcome measures. Ensure residents notes and care plans are updated to reflect any changes

Qualifications
Essential
- GCSE/NVQ level 2/3 or equivalent level of training and experience
Experience
Essential
- Understanding of the NHS and how it works
- Proven ability to work with and communicate confidently with differing levels of staffing
- Proven ability to work with external stakeholders
- Good interpersonal skills
- Ability to work well across teams
- Good time management and an ability to prioritise and work to strict deadlines
- Ability to work on own initiative without direct supervision, understanding when clinical input is needed
- IT skills and experience in the use of Microsoft Office programmes
- Coordination of care from a variety of organisations
- Communication and relationship building
- Experience in health and social care
- Experience of developing relationships with a wide variety of people
- Working and delivering outcomes with determined timeframes
Desirable
- Knowledge of the needs of older people, safeguarding and the associated legislative framework
