We are looking for new member of the clinical admin support team to join Responsive Services.

Responsive Services brings together the Intermediate CareTeam, Virtual Ward, Referral Hub and Urgent Community Response, working across East Surrey to support admission avoidance and timely hospital discharge.

You will be based within the Referral Hub, acting as the first point of contact for referrals from hospitals and community partners. Referrals often come in by phone, so you will play a crucial role in gathering vital information and ensuring referrals are passed swiftly to clinicians for triage.

Main duties of the job

What you will be doing:

  • Receiving and managing referrals via phone and email
  • Acting as a professional and reassuring first point of contact for referrers
  • Accurately capturing essential information and directing referrals to clinicians
  • Liaising with internal teams and health and social care partners
  • Helping the team deliver a responsive, efficient service in a fast paced setting

Our administration team operates 8am to 6pm, Monday to Sunday. You will work shifts as part of a rota, to ensure consistent service delivery.

Job responsibilities

The role is based in Responsive Services, the clinical admin support is the first point of contact for referrers into Responsive Services which comprises of the Intermediate Care Team, Virtual Ward, Urgent Community Response and Referral Hub.

The Clinical admin support will:

  • Manage the administration of the referral process from external and internal referrers to provide better and quicker access to clinicians and community services.
  • Provide information, advice and signposting to ensure that every service user is able to speak to the right person in the right place at the right time.
  • Support the team to facilitate patients discharge from the hospital into the community.

We work closely with our Social and Health care partners so excellent communication skills are essential and the ability to work in a fast-paced environment. The administrator is required to participate fully as a member of Responsive Services providing efficient and effective running of the administrative functions.
