Job summary
We are looking for a new or recently qualified pharmacist to join our team. This post will be an exciting challenge for someone who is highly motivated and keen to develop their clinical and professional skills in primary care. The pharmacist would be working alongside our existing clinical pharmacists and prescribing technicians to support medicines optimisation and long-term condition management within the PCN. The job will include dealing with medicines-related queries from patients and healthcare professionals. You will support the PCN with the safe and efficient prescribing of medicines, the achievement of QOF and the local prescribing and medicines optimisation scheme (PMOS)
Main duties of the job
To support the delivery of safe, effective, and patient-centred pharmaceutical care within general practice. The post holder will work alongside and under the supervision of senior pharmacists, contributing to medicines optimisation, medication reviews, prescribing support, and medicines-related quality improvement activities.
The post holder will develop their clinical knowledge and skills through supervised practice while supporting the wider multidisciplinary team in managing patients with long-term conditions and medication-related issues.

This is ideally a full-time post of 37.5 hours a week, but part time working will be considered. Salary will be dependent on qualifications and experience, but we are looking to fill this post at the equivalent of a Band 6 on AfC.
Job responsibilities
Support the delivery of medicines optimisation initiatives within the practice.
Assist with identifying and resolving medication-related problems.
Promote evidence-based prescribing and adherence to local and national guidance.
Support medication safety audits and quality improvement projects.
Clinical Support
Manage medication queries from patients, GPs, nurses, and other healthcare professionals.
Conduct medication reconciliation following hospital discharge and outpatient appointments.
Review repeat prescribing requests and make recommendations in line with practice protocols.
Assist in monitoring patients receiving high-risk medicines.
Support management of long-term conditions
Prescribing Support
Assist with reviewing prescribing against local formularies and prescribing guidelines.
Support cost-effective prescribing initiatives.
Contribute to identifying opportunities for deprescribing where clinically appropriate.
Escalate complex clinical cases to senior pharmacists or GPs as required.
Patient Care
Provide advice to patients regarding their medicines, including administration, side effects, and adherence.
Support shared decision-making and patient understanding of treatment plans.
Respond appropriately to patient concerns and medication-related enquiries.
Multidisciplinary Working
Work collaboratively with GPs, nurses, healthcare assistants, pharmacy technicians, and administrative staff.
Participate in multidisciplinary team meetings where appropriate.
Develop effective working relationships with community pharmacies, secondary care, and other healthcare providers.
Governance and Quality
Maintain accurate and contemporaneous clinical records.
Ensure compliance with relevant legislation, professional standards, and practice policies.
Participate in clinical audits, service evaluations, and quality improvement activities.
Contribute to the reporting and investigation of medication incidents and significant events.
Person Specification
Knowledge and Skills
Desirable
- Experience working within general practice or primary care.
- Experience of reviewing and managing medicines-related queries.
- Experience of working with multidisciplinary healthcare teams.
- Independent Prescribing qualification or willingness to undertake training.
- Completion of, or working towards, CPPE Primary Care Pharmacy Education Pathway.
- Experience undertaking structured medication reviews.
- Understanding of QOF, Enhanced Services, and medicines-related quality improvement initiatives.
- Understanding of clinical governance
- Understanding of clinical IT systems
Experience

- Understanding of medicines optimisation principles.
- Knowledge of current prescribing guidance and clinical governance requirements.
- Good communication and interpersonal skills.
- Ability to prioritise workload and manage competing demands.
Qualifications
Essential
- Master's Degree in Pharmacy (MPharm) or equivalent.
- Registered Pharmacist with the General Pharmaceutical Council (GPhC).
