Job summary
An exciting opportunity has arisen in the Community Diabetes Specialist Team, for an enthusiastic, innovative Nurse.
We are a friendly, forward thinking team looking for the right person to complete our team.
Main duties of the job
To act as a resource and role model in expert practice facilitating and supporting the delivery of direct specialist diabetes patient care for patients and all members of the Primary health care team, nursing and residential homes.

Job responsibilities
Be the responsible lead in diabetes service development within the locality. To evaluate service delivery, identify areas for improvement and plan future development.
To provide Specialist Diabetes Triage/ Assessment clinics within the community setting

Qualifications
Essential
- Current NMC Registration
- Evidence of continuing professional development
- Educated to Degree level or equivalent
Desirable
- Post registration qualification in Diabetes
- Evidence of Masters level studies
- V300 Non-Medical Prescribing
- Teaching and assessing qualification
Experience
Essential
- Experience of working in diabetes
- Experience of autonomous & multi-disciplinary working
- Experience of audit/research /project management
- Experience in counselling patients
Desirable
- Experience of working in a community post
- Has specialist experience in diabetes
- Experience of delivering education sessions/ programmes
- Experience of working across organisational boundaries
Knowledge
Essential
- Demonstrates a good understanding of diabetes
- Demonstrates the ability to work autonomously and analytically when interpreting patient's condition and taking appropriate action
- Shows an understanding of Government Strategy for the NHS relating to diabetes
- Shows an awareness of the importance of service development and the implementation of change.
Desirable
- Demonstrates an excellent understanding of diabetes
- Has intermediate IT skills in word processing and email

- Has the ability to teach in groups
- Has a current driving licence
