Job summary

This is an exciting opportunity for a highly motivated and enthusiastic nurse to join our community based team of Heart Failure Specialist Nurses delivering advanced practice care to patients with chronic heart failure in the Harrogate and Rural District Area.

At Band 7 the post holder will have at least 2 years experience of managing chronic heart failure patients and have completed a Heart Failure module. They will also be an Independent Prescriber.

We would also welcome applications from candidates who are willing to undertake these courses and gain experience who would join our team in a developmental role as a Band 6 HFSN with a clear competency framework to work towards Band 7.

Main duties of the job

The post holder will manage/work towards managing an identified group of patients with complex health and social care needs, using a case management model. They will have/develop highly specialist nursing knowledge and advanced skill to proactively manage patients with Heart Failure, to improve the quality, experience and choice about their care enabling them to remain at home longer.
