The diabetes team based at Kidderminster Hospital provide a wide variety of diabetes services mainly on an outpatient basis but they also support the inpatient and day case areas. You will be working as part of an autonomous team delivering diabetes care and support to people living with diabetes and their families. You will also support community services such as community nursing teams and GP practices to deliver diabetes care.
The team is part of a wider diabetes service in Worcestershire and as such the service works collaboratively. While there are consultant clinics on site, you will also hold a mixed caseload of those under Consultant care and those referred directly to the Diabetes Nursing Service so you need to have a broad range of skills and diabetes experience. Most importantly you must have enthusiasm and the desire to improve diabetes care and the lives of people living with this long term condition.

In return you will be supported to develop your skills and there will be opportunities for training and further development including the opportunity to undertake a non medical prescribing qualification.
This is a part time role working 2 days each week one of which will be Friday.
Main duties of the job
After a period of assimilation, you will be responsible for your caseload and be expected to manage your workload and clinic appointments. You will be assigned some GP practices that you are responsible for liaison with on a regular basis. You will support them with complex patients and treatment decision making. You will be supported to enhance your skills in order to be able to do this well. Your senior Diabetes Specialist Nurses locally and county wide are also there to support your development in order for you to be able undertake this type of specialist review.
The service provides diabetes structured education for type 1 and type 2 diabetes. There will be an opportunity to observe and participate in the future education courses.
The service also supports diabetes in pregnancy, both pre-existing diabetes and those that develop gestational diabetes. you will have regular contact and review of these women.
The team work closely with the paediatric diabetes service to support transition into adult services. You will participate in the monitoring and support of these patients. Many of whom are using hybrid closed loop so you will be supported to be competent in reviewing and optimising people using this type of technology.
The team is always looking at improvement in service provision and your views ands ideas will help to shape the service for the future.

Work as part of the local diabetes nursing service and the county wide diabetes service to deliver exceptional levels of diabetes care to the population of Worcestershire.
Build and maintain key working relationships with the Lead Nurse for Diabetes, Senior Diabetes Dietician, Diabetes Specialist Nurses, Administration staff, WAHT Dietitians, Patients and families, GPs and Practice Nurses, Endocrine Consultants and Diabetes Specialist Midwives as well as others involved in providing diabetes care. Participating in staff meetings and trust-wide meetings and transformation and improvement projects.
The role will be innovative and encompass all aspects of the patient care pathway, acting and a nurse advisor to support patients, their carers and health care professionals within both the in and outpatient setting. To advise both secondary and primary care staff on the management of patients with diabetes, promoting effective care and avoiding unnecessary hospital admissions/referrals.
Provide and receive complex and / or sensitive information from patients and staff. For example, breaking bad news to patients, giving complex information about treatment and their outcomes, including during pregnancy and pregnancy loss. Able to support and talk to distressed patients, visitors, and staff members.
Autonomously manage your own caseload. Triage referrals and discharge patients who are stable back to community services. Provide advice and guidance by telephone or virtually as well as seeing people with diabetes in clinic. Use technology to it's best advantage to support those eligible for NHS funding for devices within local and national guidelines.
Promote healthy living and support people with their health choices including exercise . Refer to diabetes structured education programmes and participate in such programmes when the opportunity arises. Support people with smoking cessation. and signpost to community services. Support people to access Talking Therapies when the need arrises.
Undertake agreed mandatory training and to seek out opportunities to enhance knowledge, skills and develop practice. Maintains a programme of personal and professional development by attending in-service training and utilising relevant educational opportunities such as self-directed learning and attendance of courses, conferences and seminars as identified in PDR.
Participate in the teaching and development of staff and ensure records kept of all training activities within the department. Ensure these records are sent to the Training department to maintain the ESR (electronic staff record).
Function effectively as a specialist practitioner in line with the recommendations of the Nursing and Midwifery Council and the Department of Health and operates as a member of the Diabetes Team and wider multidisciplinary teams.
Provide accurate, comprehensive contemporaneous and clear documentation of patients care. Communicate effectively to others and treatment or medication dose changes as appropriate and locally agreed. Ensure that own and others written reports are in accordance with Trust policy and NMC guidelines.
Support with audit and research within the department. Report any adverse incidents, report by yellow card submission any medication issues.
Participate in complaint investigation and reporting of any concerns raised by others about the service.
Follow trust policies and procedures for clinical and non clinical processes. Participate in writing policies and guidelines and patient information leaflets.
Risk assess the working environment and report any issues. Maintain checks on resuscitation equipment. Order supplies for the department in a timely way.

Essential
- Pass grade in GCSE or O level English and Maths.
- NMC registration.
Desirable
- Post graduate Diabetes course such as Insulin for Life, Masters in Diabetes Care, Topical, ENB 928. Or evidence of further study in diabetes care and management.
