Job summary

We have a fantastic opportunity for a Digital and Transformation Lead to work 4 days a week (0.8 FTE) to join the PCN to support the 6 core network practices, the Clinical Director, the Director of Strategy and Partnerships and the PCN Manager in the delivery of the Network DES helping to shape and inform innovative and exciting new services.

You will have the opportunity to bring your ideas to continually develop and improve the delivery of care in collaboration with partner providers to meet the local population health needs.

The core objective of the PCN Digital and Transformation Lead role is to support the PCN and its practices to deliver ongoing improvement to services and workstreams in line with achieving the DES and Impact and Investment Fund (IIF). The role will require experience in quality / service improvement, expertise in transformational change and a good understanding of general practice systems and processes, including familiarity with general practice digital systems and how they can be used to deliver improvement. Experience of using data to drive and design service improvements will also be an advantage.

The post holder will work with practices and system partners to improve patient access, experience and outcomes through practical implementation of transformation initiatives.

Job responsibilities

The post holder will work with practices and system partners to improve patient access, experience and outcomes through practical implementation of transformation initiatives.

Key Responsibilities

1. Service Improvement & Transformation

  • Support the delivery of PCN transformation priorities in line with Network Contract DES requirements.
  • When requested by the practices and if needed, work with them to identify service challenges and implement practical improvement solutions
  • Assist in redesigning pathways to improve access, efficiency and patient experience.
  • Contribute to the development and delivery of the PCN transformation plan.
  • Provide hands-on support to practices to implement and sustain agreed changes.
  • Apply quality improvement tools to support change and monitor outcomes.

2. Digital Transformation

  • Support practices to use and optimise digital systems (e.g. online consultations, clinical systems, workflow tools).
  • Assist with implementation of new technologies and digital initiatives across the PCN.
  • Promote digital inclusion and support equitable access to services.
  • Ensure digital changes align with PCN and system priorities.
  • Be responsible for leading on DPIAs and data sharing agreements

3. Data & Population Health

  • Use data, and in conjunction with the practices, to support service improvement, population health management and workforce planning.
  • Collect, analyse, and present data to highlight trends, variation, and opportunities for improvement.
  • Support consistent use of metrics and reporting across practices.
  • Contribute to monitoring and evaluation of transformation initiatives.

4. Stakeholder Engagement & Communication

  • Build effective working relationships with practice staff, PCN colleagues and system partners.
  • Engage clinical and non-clinical teams in transformation activity.
  • Support communication of changes to staff and patients.
  • Facilitate sharing of learning and best practice across practices and PCNs
  • Participate in meetings, working groups, and improvement initiatives.

5. Policy & Service Development Support

  • Support interpretation and local implementation of national and system priorities, including the Network Contract DES.
  • Assist in aligning practice activity with PCN and ICB objectives.
  • Keep up to date with relevant policies and guidance.

Person Specification

The post holder will demonstrate:

  • Experience in general practice, primary care, or a service improvement role.
  • Understanding of PCNs and the Network Contract DES (or willingness to develop this knowledge).
  • Experience supporting change, service improvement or project delivery.
  • Ability to use data to inform decisions and measure outcomes.
  • Familiarity with digital systems used in healthcare or similar environments. We use SystmOne in Boston so experience of this is an advantage but not essential as full training will be given.
  • Strong communication and interpersonal skills.
  • Ability to work independently with appropriate supervision and prioritise workload.
  • Ability to interpret and present information clearly.

Qualifications

Essential

  • Grade A - C (or 5 and above) in English and Maths GCSE
  • Experience working in general practice with a good working knowledge of practice systems, processes and procedures.
  • Experience of using programme management and improvement techniques.
  • Experience of interpreting national and/or system objectives into tangible plans for delivery.
  • Ability to extract, collate, visualise and use data and insight to both drive change and to understand and adapt programmes.
  • Excellent communication skills and an ability to engage successfully with a wide range of people at all levels within an organisation.
