Job summary

24 hours per week to cover maternity leave.

The Family Health Worker will work within the West Port Public Health Nursing Team; which includes Health Visitors, School Nurses and Family Health Workers. The Public Health Nursing service work with children and young people aged 0-19-years; this role will primarily support children aged 0-5-years.

The Family Health Worker will be accountable to the Specialist Community Public Health Nurses (Health Visitor or School Nurse) in the delivery of the Department of Health, Healthy Child Programme 0-5years.

The Family Health Worker will implement approaches to promote and help prevent adverse effects on health and well-being.

The Family Health Worker will deliver planned episodes of care and health promotion in the home and community settings.

This role may not be eligible for sponsorship under the Skilled Worker route, please refer to the Direct Gov website for more information with regards to eligibility.

Main duties of the job

Working across a variety of settings the Family Health Worker will deliver planned episodes of targeted support to families in their own homes. The Family Health Worker will also complete universal developmental reviews in clinics and nurseries. The Family Health Worker will faciliate Child Health Clinics; completing growth measurements and providing parents and carers with advice and support relating to health promotion topics.

Job responsibilities

To plan and undertake home visits to deliver an episode of care, as delegated and assessed by the Health Visitor or School Nurse in response to identified need.

The Family Health Worker will work in partnership with families, health professionals and other agencies as required.

The Family Health Worker will have the ability to communicate and liaise clearly and effectively with clients, colleagues and a variety of agencies within community settings and base.

Be competent to undertake delegated activities within the Healthy Child Programme, utilising appropriate tools when assessed as competent to do so for universal families.

To identify deviations in normal child development and refer to the Health Visitor responsible for delegation and plan of care.

In response to identified Public Health need (local and national directives) delivering and evaluating individual, clinic and group activities. Utilising evidence based approaches to target individuals and groups.

Within own competencies and knowledge, implement identified packages of care that empower and meet client need and address service requirement, reviewing effectiveness of specific activities as they proceed and modify appropriately.

To have knowledge of and act in accordance with local child protection policies and procedures.

To practice in accordance with the standards, protocols and policies which have been agreed by Livewell Southwest, demonstrating knowledge of them and how to access them.

Person Specification

Qualifications

Essential

  • Vocational Qualification Level III in childcare or health related area.
  • Maths & English GCSE Grades 4 and above or equivalent.
  • Additional qualifications in child and family related areas.

Desirable

  • IT skills.

Knowledge

Essential

  • Working with children and/or families in partnership.
  • Knowledge of normal child development.
  • Awareness of Public Health

  • Works on an individual basis with children and families, supervised by the SCPHN (Health Visitor or School Nurse), for time limited, outcome based episodes of care.
  • Is able to recognise areas of concern within families and follow agreed procedures.

Experience

Essential

  • Working with children and/or families in partnership.

  • Up-to-date working knowledge of a range of areas related to normal child development, for example, nutrition.
