Job summary
The post holder will assist with the delivery of care management to the service population and undertake tasks and duties delegated by a GP/ Nurse or other suitably qualified clinical professional strictly in accordance with service practice guidelines and protocols. This role will support a variety of services across the organisation.
This role is a full time (Monday - Friday 9AM - 5PM working days) role, with a total of 37.5 working hours per week. The wage is £13.83 per hour.
The post holder will at all times demonstrate and behave in a manner in accordance with the organisation's policies and protocols.
Main duties of the job
Key responsibilities
- New patient registration checks, NHS Health Checks, including completing any necessary paperwork and referrals within the practice
- BP Monitoring
- Urinalysis
- Phlebotomy
- Prepare and maintain environments and equipment before, during and after patient interventions
- Notify a GP/Lead Nurse/ANP of any significant patient issues
- Ensure full and accurate recording in the patients computerised records
- Carry out administrative tasks for example, but not limited to, call and recall activities and stock takes as directed by senior members of the service
- Provide lifestyle advice to patients making any necessary referrals within the practice
- Weight monitoring
- Ensure specimens are labelled and bagged ready for collection with the necessary paperwork completed
- Simple woundcare dressing
- Ensure clinical waste collection runs smoothly, reporting any problems to the nurse manager
- Ensure sharps bins are ready for collection
- Restocking / maintenance of equipment
- Restocking of clinical areas and consulting rooms

Person Specification
Qualifications
Essential
- 1.GCSE or equivalent in Maths and English Grade C
- 2.NVQ2 or equivalent experience
- 3.Knowledge of Personal Care
Experience
Essential
- 1.Minimum 1 years recent primary care and/or community health care assistant experience
- 2.Knowledge of clinical governance issues in primary care
- 3.Ability to manage and prioritise workload
Desirable
- 1.Knowledge of local and national promotion strategies, health policies and public health issues
- 2.Working as part of a multi-disciplinary team
- 3.Ability to develop relationships with internal and external stakeholders
