Essex Learning Disability Partnership are looking for a Healthcare Assistantwith a passion for delivering the highest standard of care for individuals that have a learning disability.

You will be working as a healthcare assistant within the primary care network, to obtain where the gaps fall within the primary care network and support within in to help minimise these gaps. This will include identifying areas with low covid vaccine, flu vaccine and physical health checks uptake. The job role will be based in Essex.

Main duties of the job

Main Responsibilities

The postholder will provide support to an allocated group of service users as delegated by the Team Leader or agreed registered practitioner. The postholder will be expected to work within the Community Specialist Learning Disabilty Team in line with service needs.

The postholder will receive regular supervision as appropriate to their ongoing developmental needs and requirements and partake in reflective practice and training on a regular basis.

Job responsibilities

The Role

To have regard for the physical, psychological, emotional and environmental needs of the service users and their carers

To plan and lead group and individual sessions as directed

To provide support as directed by the Nurse Team leader and in close liaison with other agencies delivering agreed outcomes.

To work with colleagues and Health Access Champions in a variety of settings to promote the uptake of AHC, access to screening and vaccination programmes. , reporting any concerns for service user health and wellbeing to the Nurse Team Leader

To keep accurate records of tasks undertaken, observation and events concerning service users and to report to other members of the team, as appropriate, and to prepare reports as requested

To maintain and promote good communication and working relationships both within the team and with colleagues from other agencies

To develop a knowledge and understanding of the specialist needs of service users, and the approaches used in their care across diverse health settings

In collaboration with Primary Care Networks, support the process of understanding and improving service user experiences of care in other health settings through data collection, observation, feedback, discussion, and reporting through the clinical team

Provide information of health promotion and healthy living as appropriate to individual need

To undertake and assist in the organisation of the day to day smooth running of the service

Contribute towards the ongoing quality of service and care and identify any risk issues in relation to the service users health and social care need

Networking and promoting positive relationships within the primary care network

Person Specification

Qualifications

Essential

  • Evidence of academic qualifications achievement
  • NVQ Level 3 or Equivalent

Experience

Essential

  • Minimum of 2 years' experience as a Health Care Assistant - working in a Health & Social Care setting.
  • Experience of working with people with a learning disability, who may have mental health needs and / or challenging behaviours
