Job summary

This role is offered on a secondment basis for 12 months. The hours are part-time, 15 hours per week.

An exciting opportunity has arisen for a suitable qualified and experienced Occupational therapist to undertake a project role within Berkeley Vale PCN in delivering a Vocational Rehabilitation service. Patients will been assessed and suitable advice and recommendations will be made including the appropriate issue of Med 3 Fit notes, recommendations on reasonable adjustments to employment and referral on to other MDT members including Employment Specialists within the team.

Main duties of the job

Run clinics within the primary care network where identified patients will be assessed and outcome measurement taken.

Create individual treatment plans including the issue of Med 3 Fit notes where appropriate and fitness to work reports

Advise and work with patients to implement reasonable adjustments to their employed role

Measure outcomes using the Work and Social Adjustment Scale

Record treatment interventions following Trust and PCN procedures and maintain HCPC standards of practice.

Communicate with other members of the MDT including employment specialists, GPs etc.

Maintain accurate and up to date data on all interventions to feed into the end of project report and local dashboards.

Job responsibilities

Highly Specialist OT JD&PS

  • Provide specialist breadth and depth of skills and knowledge to deliver evidence basedvocational rehabilitationto adults of working agewithinBerkeley ValePrimary Care Network.

  • Lead and line manage the Occupational therapy service at Berkeley Vale PCN.

  • Work as a key member of a specialist multidisciplinary servicebased within multiple GP surgeries.

  • Contributeexpert knowledge and outcomes ofoccupational therapyassessments to support the creation of specialist intervention plansand reasonable adjustments to enableadults to returnto, andsustain employment.

  • Research, develop and share current best practiceto optimise vocational rehabilitation opportunitiesand improve participation in occupation.

  • As a member of thewiderOccupational Therapy leadership group, support the continuous practice developmentofthe occupational therapy workforce

  • To hold a clinical countywide caseload

Person Specification

Qualifications

Essential

  • Degree / Diploma in Occupational Therapy
  • Evidence of relevant post graduate specialist training or equivalent experience and short courses to MSc, post graduate diploma level or equivalent

Desirable

  • Experience of vocational rehabilitation
  • Experience of working in Primary Care

professional and specialist knowledge

Essential

  • Robust knowledge of governance issues including consent, person centred planning, health facilitation, care programme approach and data protection

Desirable

  • Experience of leadership of staff
