Job summary

**This position is open to staff working at the Trust in the 'ECT Family' this includes current East Cheshire NHS Trust employees, both NHSP and ECT bank staff, ISS staff and volunteers at the Trust. Unfortunately, we cannot accept applications from applicants not listed within these groups.**

An exciting opportunity has arisen in the Hospital at Home Service in East Cheshire for a Call Handler and Care Coordinator. We are looking for a dynamic and highly organised person to join our fast paced multi-disciplinary team. The successful candidate will be responsible for supporting the Urgent Community Response (UCR) staff in processing and coordinating referrals for UCR visits, equipment provision and arranging emergency care packages with our care providers.

Hours per week : 37.5

Main duties of the job

  • To deal with in-coming and out-going telephone enquiries, relaying messages etc., where relevant.
  • To input, process and check UCR referral forms from a wide range of referrers systems.
  • To provide an efficient supportive Co-ordination service to the Hospital at Home Team.
  • To facilitate and co-ordinate complex multi agency diary appointments to ensure that patients receive a seamless service in a timely manner.
  • To work closely with Registered professionals to ensure that patient journeys following a referral to Urgent Community Response Team (UCR) are processed in a timely and efficient manner in line with nationally defined and locally agreed targets.
  • To liaise with UCR teams continually throughout the day to ensure a prompt response to referrals and communicate any changes which may require re-prioritisation of visits .
  • To be responsible for the ordering of patient equipment as directed by the Registered UCR professionals.
  • Data collection and assisting with auditing.
  • Responsible for maintaining dynamic patient databases with accuracy.
  • Produce formal letters as required.

Person Specification

Qualifications

Essential

  • Educated to GSCE/Level standard or equivalent - 5 grade C and above in relevant subject
  • Evidence of continuous personal development in a relevant area
  • NVQ level 3 education or equivalent experience

Desirable

  • IT qualifications

Knowledge & Skills

Essential

  • Detailed knowledge of community-based urgent care pathways, including Hospital at Home and the Urgent Community Response (UCR) model, to support safe and efficient coordination of patient care.
  • Knowledge of Health Service Patient Administration Systems
  • Knowledge, skills, or experience in health/social care
  • Shows insight into the needs and circumstances of patient group/vulnerable adults.
  • Excellent knowledge of Microsoft office systems
  • Ability to recognise limitations of the role
  • Understand the concept of self-care and self-management principles

Desirable

  • Demonstrable knowledge of current issues in health and/or social care including all relevant legislation and the government's agenda
  • Good working knowledge of relevant policies and local council procedures specific to the patients concerned
  • Knowledge of basic clinical procedures, observations and care.
  • Awareness of effects of ill health and the ageing process

Experience

Essential

  • Previous experience in using Microsoft 365.
  • Experience of using EMIS (Egton Medical Information Systems).
  • Experience of working in similar fast paced busy environment.

  • Experience of using EMIS

  • Excellent communication skills (Written and oral)
  • Ability to establish credibility with all levels of staff
  • Tact and diplomacy, team worker, flexibility and able to work without direct supervision
  • Proficient in the use of Microsoft office and otherIT systems and have a pleasant and confident telephone manner.
