Job summary

The Post Holder will support the delivery of the Local Navigation Hub by providing coordination and navigation for caseloads across health and social care.

Coordinate and manage the administrative functions of the Multidisciplinary meetings including minute taking and tracking actions.

The Support worker is a pivotal role to the LNH and will be the interface between primary care, secondary care, community care, social care, mental health, out of hours and voluntary organisations.

Main duties of the job

Receive and collate information from health and social care colleagues including the voluntary sector plus out of hours calls to identify if further support is required.

Signpost or refer onward to relevant service as appropriate

Contribute to assessments to identify a specific need, to maintain independence in the place they call home (own home, residential home etc).

To ensure the IT requirements for recording activity are adhered to in collaboration with other team members To support with the provision of agreed performance/activity data within the EHP data team.

About us

Erewash Health Partnership (EHP) was established on 1 April 2018 and is a single GP Partnership comprising of six constituent practices (see below). EHP employs approximately 250 people who work within the six constituent practices and within the EHP central team, which provides support services e.g., finance, operational, data & performance etc.

As well as providing high quality core primary care services, EHP is the prime provider for delivering services to Erewash Primary Care Network (EPCN), as well as operating its own services for EHP Partners.

EHP is governed by the EHP Senior Leadership Team reporting to the EHP Board which meets quarterly. The Board currently consists of one GP Partner from each constituent practice and the EHP Managing Partner & CEO, with other colleagues in attendance.

Job responsibilities

General duties

At all times the post holder is to behave in a professional way that encourages quality care, the development of a team spirit and fosters continual improvement across the PCN.

The post-holder will be expected to carry out any other duties that may reasonably be required or directed in line with the main duties and level of the role to meet the changing needs of EHP and EPCN.

Key Job Responsibilities

Receive and collate information from health and social care colleagues including the voluntary sector plus out of hours calls to identify if further support is required.

Communication and Relationships

Work with health and social care system partners to ensure referrals into the LNH are received and managed.

Develop excellent working relationships with the Care Homes, GP practices, adult social care, hospitals, pharmacists, voluntary and statutory agencies.

Fulfil an intermediary role between individuals, receptionists, administrators, clinicians, social workers, therapist and mental health teams.

Meet regularly with Community GP or designated person to review caseloads.

Help keep SystmOne up to date with relevant records for colleagues involved in care to be able to access.

Work with colleagues to ensure a point of contact is always available to case-managed or potentially case-managed individuals during working hours

Communicate effectively with service users and their families/carers, other staff both internal and external and members of the public

Communicate with other members of the integrated care system, if individuals need to access other services

Refer complex cases to Community GP as senior clinical decision maker

Manage and prioritise workload daily and deal with the competing demands of the role.

Help coordinate and manage caseloads within SystmOne and work with the Proactive use of the whiteboard or other communication tools within the locality for communication with hospital teams.

Physical Skills

Standard key board skills are required.

Use of EHP IT systems e.g., SystmOne.

Use of computer and telephone will largely be continuous.

Miscellaneous

Demonstrate a flexible attitude and be prepared to carry out other duties as may be reasonably required from time to time within the general character of the post or the level of responsibility of the role, ensuring that work is delivered in a timely and effective manner.

Qualifications

Essential

  • Good General Education.
  • GCSE English and Maths AC
  • Experience of using Systm1 clinical system

Desirable

  • European Computer Driving Licence (ECDL)

Knowledge and Skills

  • IT literate inc. experience in Word & Excel at intermediate level
