As a Neighbourhood Health Manager, you will support the delivery of the Essex Neighbourhood Health Service across Basildon and Thurrock. Working with the Senior Neighbourhood Health Manager, you will coordinate programmes that improve access, quality, integration, and outcomes.
You will work with partners across health, social care, local government, and the voluntary sector, leading programme delivery, stakeholder engagement, and performance monitoring. Using population health data and local insight, you will identify priorities, target resources effectively, and help improve neighbourhood health services across South West Essex.
Main duties of the job

As a Neighbourhood Health Manager, you will support delivery of the Essex Neighbourhood Health Service across Basildon and Thurrock, helping to improve outcomes, reduce inequalities and strengthen neighbourhood-based care. Working with partners across health, social care, local authorities and the voluntary sector, you will lead projects and service improvements that deliver local priorities.
You will use performance and population health data to support decision-making, monitor progress and improve services. Responsibilities include programme coordination, stakeholder engagement, governance reporting, budget monitoring and supporting integrated working.
About You
We are looking for an experienced and motivated individual with a Master's degree or equivalent experience and a commitment to professional development. You will have experience of delivering projects, programmes or service improvements within health, care or public sector settings.
You will be skilled in partnership working, stakeholder engagement and prioritising competing demands. You will be confident using data to support improvement and communicate information effectively to different audiences.
You will have experience of project delivery, service improvement and partnership working within health or care settings.

Job responsibilities
ROLE PURPOSE / SUMMARY
The Essex Integrated Care Board (ICB) is a new organisation formed through the consolidation of three ICB areas in response to the NHSs ambition to deliver a more streamlined, strategic and cost-effective model of health commissioning. The ICB will oversee the commissioning of care for the approximately 2 million people who live in Essex. Operating within a reduced running cost envelope and aligned to the new requirements of ICBs in England, the ICB will focus on improving population health outcomes, health equity and smarter resource allocation.
The Neighbourhood Health Manager supports the coordination and delivery of the Essex Neighbourhood Health Service for specific Multi-neighbourhood area(s). The post holder translates strategic and operational plans into practical delivery, working with local partners to improve access, quality and integration of care. They play a key role in coordinating local initiatives, monitoring performance, and ensuring that delivery activity aligns with the priorities agreed by the Senior Neighbourhood Health Manager.
The role provides local leadership and coordination across primary, community, mental health, social care and voluntary sector partners. The post holder will manage a small team of officers and ensure that local programmes and partnerships deliver tangible improvements in population health outcomes and patient experience.
As the ICB transitions to become a leaner, more focused strategic commissioning organisation the post holder will be expected to support the Senior Neighbourhood Health Manager in delivering on the three fundamental shifts set out in the NHS 10-year plan:
- From Treatment to Prevention: Embedding preventative, personalised and population based approaches to commissioning.
- From Hospital to Community: Championing care model redesign that supports community and neighbourhood health.
- From Analogue to Digital: Enabling clinically driven transformation through digital innovation, data analytics and evidence based improvement.
KEY DUTIES AND RESPONSIBILITIES
Policy and service development
- Support implementation of the Essex Neighbourhood Health Service across defined neighbourhoods.
- Contribute to the development and implementation of local delivery plans aligned with ICB and system priorities.
- Support the introduction of new care models and integrated service pathways.
- Promote prevention, personalization and innovation in service development.
- Ensure community insight and patient voice inform local service planning.
- Ensure compliance with relevant ICB policies
- Use research and innovation to inform service improvements or performance management
Strategy and analysis
- Use of complex health data sets, including those relating to the performance, quality and activity of neighbourhood health services to inform decision making
- Support performance monitoring and reporting to the Senior Neighbourhood Health Manager and relevant governance forums.
- Identify service variation and support the implementation of improvement plans.
- Turning analysis into useful insights that can be used to inform decision making and delivery
- Present complex data, information and insights, including qualitative and quantitative data, through clear and simple written and verbal briefings
- Use of Population Health Management methodologies and tools to inform projects and programmes that will improve outcomes for patients and sustainability of health services
- Contribute to the evaluation of pilots, projects and transformation programmes.
Planning and organisation
- Coordinate delivery of local programmes, projects and improvement initiatives.
- Maintain programme documentation including project plans, risks logs and delivery trackers.
- Develop and deliver project plans relating to Neighbourhood Health Senior Officer, ensuring appropriate governance, risk management, escalations and reporting
- Manages competing priorities, prioritizing own and team resources to effectively deliver on short, medium and longer term priorities
- Ensure accurate reporting and records management
- Prepare reports, dashboards and briefings for boards, committees and stakeholder meetings.
Communications and Partnerships
- Work collaboratively, modelling inclusive, respectful and compassionate leadership
- Able to take account of a broad range of perspectives, including staff, patients and the public, to inform decision making
- Present information and training to large groups
- Build and maintain effective relationships with Primary Care Networks, community providers, local authority teams and VCSE partners.
- Act as a key point of contact for local stakeholders, ensuring smooth coordination across neighbourhood teams.
